Here is what you'll need to make your visit with us run smoothy:

1. A completed new patient form (this can be filled out in our office, or you may print out a version and fill it out prior to your visit. To view and print a copy, click the "New Patient Form" button to the left).

Please note: You will need the FREE Adobe Reader to view and print any forms. To get the Reader, click on the button to the left

2. Please review our Notice of Privacy Practices and complete the Acknowledgement of Reciept of Privacy Policies form (this too can be viewed and filled out in the office or completed prior to your visit. To see our privacy policy and complete the forms at home, click the appropriate buttons to the left).

3. Make sure that you have the proper referrals from your primary doctor PRIOR to your visit with us.

4. Please make a list of your current medications and the names of medications you have tried for your skin condition prior to your visit with us. This will help us greatly to give you safest and most accurate treatment.

5. Please bring a copy of your insurance card with you to your visit.
