ALEX T. HIRSH, M.D.

Dr. Alex Hirsh is a board certified dermatologist and member of the American Academy of Dermatology. He completed his specialty training at Northwestern Memorial Hospital where he was Chief Resident in Dermatology in 2001-2002. Dr. Hirsh completed his medical school training at The Ohio State University College of Medicine where he was a member of the Alpha Omega Alpha Medical Honors Fraternity and graduated Magna Cum Laude in 1998. His undergraduate studies were completed at the University of Pennsylvania where he was a member of the Beta Gamma Sigma Honor Fraternity and graduated Summa Cum Laude in 1994. Dr. Hirsh has special interests in general dermatology, pediatric dermatology, dermatological surgery and ultraviolet light therapy.

FRED S. HIRSH, M.D.

Dr. Fred Hirsh is a board certified dermatologist and member of the American Academy of Dermatology. He is the past president of the Cleveland Dermatologic Society and an Associate Clinical Professor at Case Western Reserve University. He completed his specialty training at Case Western Reserve, University Hospitals of Cleveland where he was Chief Resident in Dermatology in 1975-1976. Dr. Hirsh completed his medical school training at The Ohio State University College of Medicine where he was a member of the Alpha Omega Alpha Medical Honors Fraternity and graduated Cum Laude in 1972. His undergraduate studies were completed at the Ohio State University where he was a member of the Phi Beta Kappa Honor Fraternity and graduated Cum Laude in 1968. Dr. Hirsh has special interests in general dermatology and dermatological surgery.

Discussing your personal health information, making sure it is accurate and up to date and entering it into your electronic medical record will require extra time the first visit you have with us as we transition your chart into its electronic version.

If you are unable to complete our form at home or use the online portal, we ask that you arrive 30 minutes prior to your visit to allow adequate time to update and enter your information into our new system.

If you are looking to save time during your visit, you can fill out the necessary forms in the comfort of your own home. This will allow you time to get all of the necessary details together and to prepare the forms prior to your arrival in our office.

With the online portal, you will have direct access to your electronic medical record in order to supply and verify your personal health information. After gaining access to the portal, you will directly input your health information including your past medical history, allergies and current medications.

We are excited to offer you a new opportunity to get your test results either online or by phone.

This is our latest effort to improve doctor/patient communication as well as closely following the new HIPAA guidelines for patient privacy and security.

By giving you a unique user ID and PIN number, as well as the ability to change your PIN at any time, you are assured that you, and only you, will be the person to receive your test results.
