If you have not been seen in our office for over 3 years, please refer to the "New Patients" page to prepare for your visit.

Here is what you'll need to make your visit with us run smoothy:

1. If you have not done so in our office to date, please review our Notice of Privacy Practices and complete the Acknowledgement of Reciept of Privacy Policies form (this can be viewed and filled out in the office or completed prior to your visit. To see our privacy policy and complete the forms at home, click the appropriate buttons to the left).

Please note: You will need the FREE Adobe Reader to view and print any forms. To get the Reader, click on the button to the left

2. Make sure that you have the proper referrals (and that they are current and up to date) from your primary doctor PRIOR to your visit with us.

3. Please make a list of your current medications. This will help us greatly to give you the safest and most accurate treatment.

4. If you have a new insurance carrier, please bring a copy of your insurance card with you to your visit.
